Necessary Information for New Patients

Here are some tips that might be useful when you visit us for the first time.
  • You can also fill out the form by downloading the new patient paperwork from our website. You can also pick it up at the clinic.
  • Please arrive 15 minutes prior to your scheduled appointment so that the paperwork can be systematically processed.
  • Bring a copy of your insurance card and picture ID when you're visiting us for the first time.
  • A copy of the record of any immunizations that your child might have received in the past is also required. It helps in selecting accurate vaccines.
  • Your child will be examined only in the presence of a parent or legal guardian or a person holding a notarized power of attorney form. The appointment will otherwise be rescheduled. Only the parents or legal guardian of a child can schedule the appointment. 
  • Please pre-check with your insurance carrier for information on current benefits and hospital or lab preferences. Such information includes details on immunization coverage, preferred lab or radiology locations for diagnosis, and hospitals included in case of emergency services.
  • If your appointment is delayed by 10 minutes or more, we'll reschedule it to the nearest slot available. But if you don't make it to our office for your first appointment, we won't be able to reschedule your visit. You will need to pay a $25 no-show fee as well. 
  • The same $25 no-show charge is applicable if you cancel your visit less than 24 hours in advance. On your two consecutive failures to show up, you'll receive a warning letter and a termination letter on three failures.
